Persian Abstract
زمینه و هدف: کنترل درد بعداز جراحی توراکس یک مشکل عمده است. درد بعداز عمل، سبب آتلکتازی، هیپوکسمی، عفونت ریه وآسیب دائمی ریه می شود. هدف از مطالعه حاضر، بررسی تأثیر بی دردی بوپی واکایین (IP) جهت کنترل درد بعد از توراکوتومی می باشد.روش بررسی: این پژوهش به صورت کارآزمایی بالینی دوسوکور آینده نگر بر روی سی بیمار کاندید جراحی الکتیو توراکوتومی پوسترولترال انجام گردید. بیماران در دو گروه پانزده نفره تقسیم گردیده و سپس هر 4 ساعت به مدت 24 ساعت محلول مورد مطالعه (بوپی واکائین 5/0 % یا نرمال سالین) را به صورت اینترپلورال دریافت نمودند. تمامی بیماران جهت کنترل بهتر درد، یک میلی گرم مرفین هر پنج دقیقه توسط سیستم PCA (کنترل دوز توسط بیمار) دریافت نمودند. ارزیابی شدت درد توسط نمره دهی مقایسه ای بینایی (VAS) و میزان نیاز به مورفین از طریق PCA انجام گردید.یافته ها: تفاوتی بین دو گروه از جهت میانگین مصرف مرفین وجود نداشت. میانگین شدت درد بر اساس مقایسه VAS دردوگروه مشابه بود. بعداز تجویز محلول اینترپلورال (هم بوپی واکائین و هم سالین) شدت درد در بیماران کاهش یافت که کاهش درد توس تزریق اینتراپلورال بوپی واکائین مشابه اثر پلاسبو (نرمال سالین) بود .هرچند تکنیک بی درد اینتراپلورال در گاهش درد بیمارانی که توراکوتومی پوسترولترال شده اندموثر نمی باشد اما جنس وBMI در میزان شدت درد بعداز عمل تأثیر داشتند . بیماران زن و افراد با وزن بیشتر از شدت درد بالاتر برخوردار بودند. اگرچه اهمیت بالینی این نتیجه ارزش اندکی دارد. نتیجه گیری: تأثیر تکنیک بی دردی رژیونال اینترپلورال در کاهش شدت بعد از عمل بیماران توراکوتومی مورد اختلاف نظر است. این مطالعه بیانگر فقدان مفید بودن آنالژزی اینتر پلورال در کنترل درد بعد از توراکوتومی است.

Investigating the Efficacy of Interpleurl Regional (IP) analgesia in Surgery Cases
Abstract
Background: The management of post-thoracotmy pain is an important issue and may lead to atelectasis, hypoxemia, pulmonary infection, and permanent alveolar damage. The purpose of the present study was to investigate the efficacy of Interpleurl Regional (IP) analgesia with Bupivacaine after thoracotomy for pain control . Material and methods: Thirty patients undergoing elective posterolateral thoracotomy were included in a prospective randomized, double blinded trial. Patients were divided to equal groups and each group assigned to receive either 0.5% bupivacaine or saline solution interpleurally every 4h for 24h postoperatively. All patients also received patient-controlled analgesics (PCA) with 1mg morphine every 5 minutes for additional pain control. Pain was measured on the basis of PCA requirements and by using a Visual Analog Scale (VAS). Findings: Visual Analog Scale scores and PCA requirements Were the same for both groups . Both interpleural bupivacaine and saline significantly reduced pain scores after the administration. It seems pain reduction by interpleural instillation of bupivacaine is the result of the placebo-like effects however interpleural regional analgesia is not effective in patients undergoing lateral thoracotomy. Sex and BMI were shown to influence postoperative pain intensity. The female patients, and those with higher body mass, reported higher pain scores. This observation appears to be of only marginal clinical significance. Conclusion: The efficacy of Interpleural Regional Analgesia to reduce postoperative pain intensity in patients after thoracotomy is controversial. The present study demonstrates a lack of efficacy of interpleural regional analgesia.
