Persian Abstract
زمینه و هدف: مقاومت به آنتی بیوتیک های کینولون به عنوان یکی از عوامل درمانی اشریشیاکلی های عامل ادراری در جهان در حال افزایش است. بنابراین این مطالعه با هدف بررسی مقاومت به کینولون ها در ایزوله های بالینی اشریشیاکلی جدا شده از عفونت ادراری افراد بستری در بیمارستان های شهر تهران در سال 1396 انجام شد. روش ها: در این مطالعه تعداد 150 نمونه اشریشیاکلی از ادرار بیماران مبتلا به عفونت ادراری و بستری در بیمارستان های شهر تهران جمع آوری شد. بررسی مقاومت به آنتی;بیوتیک های کینولون با روش دیسک دیفیوژن انجام گردید. پس از تخلیص ژنوم ایزوله ها با روش فنل- کلروفرم، تکثیر ژنهای qnrA، gnrB و qnrS با روش واکنش زنجیرهای پلیمرازی انجام شد. نتایج: مقاومت ایزوله ها به آنتی بیوتیک های نالیدیکسیک اسید، سیپروفلوکساسین، نورفلوکساسین و لووفلوکساسین به ترتیب 76، 48، 30 و 18 بود. نتایج تکثیر ژن های qnr نشان داد که 40 درصد از ایزوله ها حداقل یکی از ژن های qnrA، gnrB ویا qnrS را حمل می کنند. از بین 60 ایزوله دارای ژن qnr، 73/3 دارای ژن qnrS، 23/3 دارای ژن qnrB و 3/4 دارای ژن qnrA بودند. آنالیز آماری نشان داد که بین حضور همزمان ژن های qnrB و qnrS و مقاومت همزمان به نالیدیکسیک اسید و سیپروفلوکساسین ارتباط معنی داری وجود دارد. نتیجه گیری: مقاومت قابل توجهی به آنتی بیوتیک های کینولون دار در بین ایزوله های اشریشیاکلی عامل عفونت ادراری در بین بیماران بستری در بیمارستان های شهر تهران وجود داشت که این مقاومت با شیوع نسبتا زیادی از ژن های مقاوم همراه بود. پیشنهاد می گردد مطالعات بعدی در رابطه با منشاء عفونت این نمونه ها انجام گردد.

The phenotypic and genotypic evaluation of resistance to quinolone antibiotics in clinical Escherichia coli isolated from urinary tract infection of hospitalized patients in Tehran, Iran in 2017
Abstract
Background & Aim: Antibiotic resistance to quinolones as a therapeutic agent of urinary tract infection in increasing in the world. Thus, this study was designed to evaluate the resistance to quinolone antibiotics in Escherichia coli isolated from urinary tract infection of hospitalized patients in Tehran, Iran in 2017. Methods: In the present study, 150 E. coli isolates was collected from urine of hospitalized patients with urinary tract infection in Tehran, Iran. Evaluation of resistance to quinolone antibiotics was done by disk diffusion method. After genome extraction with phenol l& chloroform method, amplification of qnrA, qnrB and qnrS genes in the isolates was performed by PCR. Results: The rate of resistance to antibiotics nalidixic acid, ciprofloxacin, norfloxacin and levofloxacin in these isolates was 76, 48, 30, and 18. The results of amplification the qnr genes showed that 40 of isolates harbored at least one of the qnrA, qnrB and qnrS genes. Among the 60 isolates harboring the qnr genes, 73.3, 23.3 and 3.4 had qnrS, qnrB and qnrA genes, respectively. Statistical analysis of results showed that there was a significant relation between the co-presence of qnrS and qnrB with co-resistance to nalidixic acid and ciprofloxacin antibiotics Conclusion: In this study, a significant rate of resistance to quinolone antibiotics was observed in E. coli isolated from patients with urinary tract infection in Tehran hospitals that showed a remarkable rate of resistant genes. It is suggested to study the origin of these infections in the collected samples in the future.
