Persian Abstract

زمینه و هدف: امروزه نبود محافظت مالی در سلامت به عنوان بیماری نظام های سلامت شناخته شده است. روشن ترین نشانه آن این است که خانوارها نه تنها از بار بیماری ، بلکه از بار ناشی از نابودی و فقر اقتصادی و به عبارتی دیگر مواجهه با هزینه های کمرشکن و فقر ناشی از تأمین مالی سلامت خود رنج می برند. پژوهش حاضر به سنجش هزینه های کمرشکن خدمات سلامت و  شاخصه های اثرگذار در این زمینه پرداخته است. روش بررسی: این پژوهش یک مطالعه مقطعی بود که اطلاعات مربوط به 384 خانوار تحت پوشش دانشگاه علوم پزشکی تربت حیدریه جمع آوری گردید. ابزار گردآوری داده ها  پرسشنامه محقق ساخته ای بود که روایی و پایایی آن تایید گردید. بروز هزینه کمرشکن خدمات سلامت بر اساس تعریف سازمان جهانی بهداشت محاسبه گردید و  عوامل مؤثر بر این هزینه ها با استفاده از نرم افزار  Stata  نسخه 11 مورد بررسی قرار گرفت. یافته ها: میزان مواجه خانوارها با هزینه های کمرشکن سلامت برابر با 77/6% بود. استفاده ازخدمات بستری بیشترین سهم در میزان پرداخت از جیب خانوارها داشت و با بروز هزینه های کمرشکن سلامت رابطه معنا دار داشت؛ همچنین ارتباط بین بعد خانوار، وجود فرد 65 سال به بالا، استفاده از خدمات دارویی و تشخیصی با هزینه های کمرشکن سلامت معنی دار است. نتیجه گیری: استفاده از خدمات مربوط به سلامت، وجود فرد زیر 5 سال در خانوار و زیاد بودن بعد خانوار باعث افزایش مواجه خانوارها با هزینه های کمرشکن خدمات سلامت می شود. این متغیرها باید جهت دستیابی به هدف برنامه پنجم توسعه کشور که 1% بروز هزینه های کمرشکن سلامت است مورد توجه سیاستگذاران شهرستان قرار گیرد.

Estimates of Catastrophic Health Care Expenditures on Families Supported by Torbat Heydariyeh University of Medical Sciences in 1391

Abstract

Background: No financial protection in health has been recognized as a disease in healthcare systems. Some families suffer from poverty, disease, and catastrophic expenditure .In this study we have tried to measure the catastrophic expenditures of health services and effective indicators on them. Materials and Methods: This was a cross sectional study and the information of 384 families who were supported by Torbat Heydarieh University of Medical Science were collected.        Data collection tool was a questionnaire the validity and reliability of which were confirmed. Incidence of catastrophic health care expenditures were calculated based on WHO methods, and Stata version 11 software was also used for data analysis. Results: The rate of households facing catastrophic healthcare expenditures was 6.77 %. The use of daycare services had the highest proportion of the expenditure which had a significant relationship with the occurrence of catastrophic healthcare expenditures. Catastrophic healthcare expenditures had a significant relationship with the number of family members, the existence of a 65 or older family member, he use of medicines, and diagnostic tests. Conclusions: The use of health services, the existence of children under the age 5 in the family, and large family size increase catastrophic health expenditures. These variables should be taken into consideration by policy makers to have an agenda in Fifth National Development Plan, aiming to achieve 1% incidence of crippling cost of health.
